multidisciplinary
Cardiac
Rehabilitation
and Prevention
Program
provides the
highest quality
of care
available, from
exercise
programs to
smoking
cessation
strategies and
cholesterol
management. .
We provide each
patient with:

by a
cardiologist

Exercise
testing

Supervised
exercise
training

Personal
nutrition
evaluation
and weight
management

Cholesterol
screening
and
management

Smoking
cessation

Risk-factor
modification
and
education

Follow-up
maintenance

Periodic
reports to
referring
physicians.
The
program
serves
to help
prevent
the onset
or
worsening
of
cardiovascular
disease by
evaluating
and
managing
high
cholesterol
and blood
lipid
levels.
Our
multidisciplinary
staff
includes
physicians,
nurses,
and
nutritionists
from the
Sections
of
Cardiology,
Endocrinology
and
Nutrition.
Our staff
members
work
closely
with each
patient's
primary
care
physician.
Patients
receive an
individualized
program of
medical
therapy,
nutritional
counseling,
exercise
and
education.
Cardiomyopathy/Heart
Failure
Our
multidisciplinary
program
provides
comprehensive
diagnostic and
therapeutic
services to
patients with
all types of
cardiomyopathy
and heart
failure,
ranging from
silent cardiac
enlargement to
advanced
symptomatic
heart failure.
The full range
of treatment
alternatives
includes:

Assessment
of
risks
and
design
of a
complete
medical
program
for
any
patient
with
impaired
cardiac
function.

Re-evaluation
of
therapy
for
patients
with
recurrent
symptoms
or
frequent
hospitalization.
-
New
investigational
drugs
and
devices.
-
Cardiac
resynchronization
therapy
with
biventricular
pacing.
-
Evaluation
and
therapy
of
associated
dysrhythmias.
-
Evaluation
for
complex
surgical
procedures
to
improve
cardiac
function.
-
Evaluation
and
support
with
univentricular
and
biventricular
mechanical
assist
devices,
as
bridge
to
transplant
or
destination
therapy.

Evaluation
for
heart
transplantation.
Referral
may
be
initiated
by
patients,
primary
care
physicians,
or
cardiologists
either
for
outpatient
evaluation
or
inpatient
hospital
transfer.
Subsequent
care
will
include
assessment
by
advanced
heart
disease
faculty
and
heart
failure
nurse
specialists,
with
continued
involvement
as
requested
by
the
patient
and
referring
physician.
